Prior to your scheduled appointment, if you’re able, please fill out this patient form online with all your current information. This will speed things up when you come into the office!

It is important to bring your dental insurance card in with you. It’s also important for us to have a list of any/all medications you’re currently taking. If you’ve seen a different dentist in the past, it’s helpful for us to have that dentist’s contact information (name, address, phone, email) so that we can track down your records and X-rays for reference.

Platte Valley Dental submits to all insurances and are in-network with the following:

  • Delta Dental
  • Blue Cross Blue Shield
  • Metlife
  • Cigna

If we are not in network with your insurance we are still happy to provide you with an estimate of your out of pocket cost, and are happy to communicate with your insurance directly.

Yes. Along with accepting most major dental insurance, we also offer financing if you need to pay your bill a little at a time. Even if you have dental insurance, you will still have out-of-pocket expenses because of co-payments, deductibles, and items that insurance doesn’t cover, like cosmetic procedures. For these expenses, we accept:

  • Cash/Check
  • Credit Cards
  • CareCredit®
  • HSA

Your health is our first priority so, if you have trouble paying a dental bill, don’t hesitate to ask us about our financing options.

Here at Platte Valley Dental we have invested in our patients' comfort which includes a new technique for administering anesthetic, offering nitrous oxide (laughing gas), and having a TV with headphones to allow patients to drown out the noise.
